The approved forms are as as follows.
1. PMKeYS.
2. If PMKeYS is not avaliable, form AF088 - Application for Leave.
5.7.4B Limit on the amount of parental leave
5.7.4C Dual serving parents
A dual serving couple whose child is born or joins their family on a permanent basis, on or before 12 February 2025 are eligible for a combined total of 18 weeks of parental leave that was applicable at the time as provided under Schedule 6 of Defence Determination, Conditions of service Amendment Determination 2024 (No. 13).
